Okay, found it.

My main video was a bit messed up.
It was a PAL 720x576 23,9 format and that had to be a frame rate of 25.

Main thing was my multiple audio stream. I noticed that I can not add an 
extra stream in Qdvdauthor. The stream does simply not get assigned with 
an extra stream number (1, 2, 3 etc.). This way Qdvdauthor complains 
about editing the master audio stream and thus not allows it. I injected 
the two stream manually with Mplex.

Furthermore adding subtitles did not went well so I had to do that 
manual with spumux. In Qdvdauthor I choose a solid colour white, and 
outline set on 1 color black, and outline transparency on 0 (zero). This 
resulted in a .png bigger than spumux can handle. In stead I used the 
following settings: solid colour white, and outline set on 2 colour 
black, and outline transparency on 25. For some reason this setting 
worked well.

So in the end it turned out not to be an error in the submenu but in the 
main video.

_Mplex your video:_
First save the audio stream separate from the video. For this see this 
howto:

Than add the streams together with Mplex: mplex -f 8 -o output.m2v 
video.m2v track-1.mp2 track-2.ac3
Note that you use -f 8 and not -f 9, as -f 8 adds empty VOBU's which you 
need in Qdvdauthor.

_Spumux two subtitle streams:_
export VIDEO_FORMAT=PAL; spumux -s 1 subtitle_0.xml < input.m2v > 
output-with-first-stream.m2v
export VIDEO_FORMAT=PAL; spumux -s 2 subtitle_1.xml < 
output-with-first-stream.m2v > output-with-two-streams.m2v

Note that the first command (export VIDEO_FORMAT=PAL;) must be given in 
order to tell spumux with what kind of format your working. I am working 
with Kubuntu so maybe with other distro's this would not be applicable. 
To make Qdvdvauthor run properly I have to start the application like 
this: export VIDEO_FORMAT=PAL; qdvdauthor
