Hi,
  I can reliably get LVM/mdraid to trigger a 'rescheduling sector'
and mark one half of my raid bad when running the iotests.
(e.g. iotest 175).
The raid refreshes and reads fine; and I've boiled it down
to a single O_DIRECT read, which in some cases turns into a BUG.

I've reported it at the kernel bugzilla;

Note there's a ~10 year old bug as well where others
have reported RAID corruption triggered with qemu in some
cases, which is https://bugzilla.kernel.org/show_bug.cgi?id=99171
