On Mac OS X host Windows XP does refuse to install on a qcow image but raw images work. I haven't tried, but I think it's the same with Vista. Installing in a raw image works fine.

Vista is well known for their problems on formatting step with a
variety of emulators.
In their developer's page they say to format the virtual disk first
with XP for use of vista under VPC.
