This allows to hotplug/unplug PHBs. I could successfully test:
- using in-kernel XICS, emulated XICS and XIVE
- hotplug/unplug with e1000 device to validate LSIs
- hotplug/unplug with virtio-net device to validate MSIs
- some simple migration scenarios
