I can see either of the following two problems:
* CRS mismatch
* wrong service URL
In the QGIS WMS connection dialogue there is a checkbox telling QGIS to
ignore the service URL advertised by QGIS server. It may be worth
checking this option.
Do you see a request in the Apache access or error log?
