AFAIK the mxd don't host the data.
So the plugin simply take from the mxd the path of the datasets and write a new qgis project from scratch adding tha same dataset with their paths.

This is pretty simple for dataset as shapefile.
It work also for data host in arcsde or in geodatabases ?
