are you opening as a delimited Text or something else?
What is the file extension you are using?

What format does QGIS think it is opening??

But if you “only” has a csv with 3 col x,y, and z – you can take Layer>Add
delimited text layer

Then make an interpolation and get your rater.
I'm able to open xyz files without headers as rasters. QGIS Master, GDAL 1.10.1.
