it would have been better to answer to the list, so that others can benefit from the answers too.

There are many bugs related to the Print Composer and the OpenLayers plugin. So I adivise you to switch to the QuickMap Services plugin, and see if the problem remains.

Personally, I do not use the print composer, so I can't help on matters related to it.

The project CRS is EPSG 3857 as I'm using OpenStreetMap as the base.

I kept this layer as EPSG:3857 and then loaded the Print Composer. I
then selected my custom grid (exactly the same as EPSG:27700 to test
it and then amend it to my local grid when it works). I do have on
the fly projection turned on too. The x and y formats are 10000.
However, the grid lines aren't in 10km spacings (they seem about
8km), they're exactly horizontal and vertical. Take London as an area
that I'm showing in the Print Composer, x=0 passes straight through
the 0 degree latitude and the northing running roughly through London
(along the Thames) shows as y=6700000.00.

Just to reconfirm, the test local grid information is exactly the
same as EPSG:27700. That's the puzzling bit, EPSG:27700 works
perfectly, but the local grid with the exact same information does
not.

Hope that provides a bit more information, would be easier to provide
an image but not sure how. Many thanks!

You have to add some more information to make the problem
understandable:

What source CRS does your data have? Did you assign the custom
projection, or reprojected the data to it? What extent does your data
have in the Custom CRS? Why can't you use EPSG:27700? Can you give
sample coordinates of what you got and what you want to get?
