I think the other culprit can be "Tile Map Scale Plugin" that zooms at certain 
tile levels

Sounds like you've probably got the OpenLayers plugin installed.
Remove it and either:

1. Use QuickMapServices instead
or
2. Use the cool new "XYZ Tile Services" feature in QGIS 2.18
