Andrew,
Maybe what you need to do is Filter the data before editing, removing all but 
the data you are looking to edit.
Another thing you can try in the Attribute Table is Show Features Visible On 
Map, I have not tried it but, you can set this as the default behaviour by 
going to Settings, Options, Data Sources and under Feature Attributes and 
Tables set the Attribute table behaviour to Show Features Visible on Map.

I'm afraid we are quite well indexed. I'd also expect slow performance on the 
map rendering if that were the problem.

Unless of course there is some client side data caching from postgres that I am 
not aware of?
