That is usually because you have no permission ;-)

What I mean is that the default location that QGIS tries to save it is a place 
where you are not allowed to save anything. Eg. on MacOSX that is in the System 
folders, probably Win and Linux have similar settings. The solution is to press 
the little button with “...” next to the File Name box and choose another 
directory to save your file.
