Hi Jochem,
it looks like the issue may be related to the Microsoft Visual C++
Redistributable version installed in your (and my) Windows system.
Could you please indicate the version of the latest Microsoft Visual C++
Redistributable installed in your system?
To workaround the issue, you could copy the file msvcp140.dll from
apps\bin to apps\qt5\bin.
Then you should launch the Qt Designer normally from the shortcut link
icon "Qt Designer with QGIS ... custom widgets".
