I have always had trouble with formatting under SMSQ/E. The work-around I use is to format the disk under DOS, and then use a program (such as my QXLFormat) that can rewrite the directory on the disk to be a QL format one. Also useful for setting previously formatted QDOS format disks back to an empty state.
