The problem usually is one of the following: either you are requesting
tiles that are outside the covered area for the WMS, or the zoom level is
to close to the surface, and the WMS doesn't have tiles for that close zooms.

In either case QLGT sends the requests to the remote WMS, but the server
just sends a 404 HTTP error, an HTML page, or empty tiles. If you are used
to tcpdump or tools like that, you could capture traffic on your network
interface to check the exact dialogue between the QLGT (client) and the WMS
(server). Or just send to the list the exact location, zoom level and link
to the WMS XML definition file so we can check ourselves.
