Well, I did some testing and finally I have almost discovered the problem.

I have 6 smtp/pop/imap servers behind a lvm balancer, and I still had one of 
them running the version 20031001 of the patch.

When a message comes in through the older SMTP, there is some issue with the 
file name/permissions/date (I don't know exactly what), that doesn't allow 
the new POP daemon to see it.

And, when a message comes in through one of the new SMTP servers, the older 
POP daemon doesn't tag it as already downloaded, resulting in duplicates.
