Set the envvar LOGLEVEL to 255 for qmail-start and restart it. e.g.
echo 255 > /var/qmail/boot/qmail/env/LOGLEVEL
svc -t /var/qmail/boot/qmail
if you are using our startup scripts.

This is the error you get when you inject mails without recipients. e.g
by a badly configured qmail-group.
