I like to run clamav via qscanq. Qscanq is written in C and is nearly 10 times faster that qmail-scanner.pl in some load tests I have run.
The qmail-queue patch is included in the qmail-ldap patch, you have to add "-DALTQUEUE " to LDAPFLAGS in the Makefile.
