You will need to use something like qmail-scanner.pl or qscanq. I use the
latter, much faster than the perl script. I was able to get qscanq to work
with csav, though it was a bit of trouble, I used this for the first line
of conf-scancmd:
/package/mail/qscanq/command/run-antivir

I would consider using clamav instead, csav got so behind on virus
definitions, and they were never able to scan password protected .zip
files, which clamav does nicely. And, clamav is free!
