I've gone so far as to using ktrace (or strace or truss), depending
on the OS in question) to verbosely dump out what the executable
is doing.

That may reveal some 'silent errors', like a system call failing,
trying to access some resource that is missing.

Specifically to SMTP AUTH, I ran into grief wherein there are
different patchsets out there, with different versions of said
patches, which result in different invokations of qmail-smtpd, with
different argument lists.

There seems to be a juggling match as to whether or not you need
the the hostname, or /bin/true, or whatever.  (This was _not_ a
qmail-ldap installation, I'll point out.)

In any case, carefully review your SMTP AUTH patchset's example
code as to how to construct the qmail-smtpd invokation.  If you're
confident you have that correct, but can't get it work (and can't
get meaningful diagnostics), you can wrap the qmail-smtpd call with
with ktrace (or whatever) to reveal all sorts of details...
