I would like to ask if it's necessary to install all previous patches if I> want to install the last one or not. I expect that no but I would like to be sure with it.
You expect correctly. You must only apply the latest patch on a vanilla qmail-1.03.tar.gz distribution.
I already have some patch from year 2004 installed but I would like to

Use a "fresh" extracted qmail-1.03 and apply the latest patch on this directory as described in the qmail-ldap documentation.
Tweak the Makefile as necessary and run "make setup check". Then the new binarys will be installed. A backup of the old would be wise.
The new run-files must adapted manually.. (or you delete the old and then the new will be installed by 'make setup check'.. ) But often this files are modifiedby one's own hand so be carefull!
