Connection limit on the OpenLDAP side. You have up to 150 qmail-lspawns running all of them opening a session to the OpenLDAP server. Then you have to take into account the sessions of auth_* and qmail-verify. In other words your OpenLDAP server needs to handle more than 300 concurrent sessions or some sessions may get dropped because the OpenLDAP server is overloaded.
