Not to yank my own chain, but clamdmail includes clamd-greylist, which is a qmail-queue front-end to do greylisting against a postgres database. The greylisting algorithm is pretty simple (I just take the whole list of address on fd1) but it's been working well for me, and the greylisting policy is highly flexible.
