Few time ago I saw an email reporting problems using Qmail and new OpenLDAP 2.3.x serie. I don't know if this problem is related with yours or if the previous version problem has been solved in recent qmail-ldap patch release. ¿Why dont you try using OpenLAP 2.2.x instead of 2.3.x?
