You need to create groups nofiles, qmail and vmail.
Also you need users: alias, qmaild, qmaill, qmailp in nofiles group and
qmailq, qmailr, qmails in qmail group etc.
Did you miss some of these steps?

What does your /var/qmail/bin/qmail-showctl say?
