Personally i use qmail-smtpd tls/ssl patch.

when you connect to servers with TLS support, they send after "ehlo" a list
of supported command (STARTTLS,AUTH...).
 If you use tcpserver TLS patch, i think that smtpd will not handle TLS.

That i understood : tcpserver/SSL  you make an encrypted tunnel, with
qmail-smtpd patched, TLS is handle by the protocol smtp(RFC2487).
But you can try and see if smtpd can be used in an encrypted tunnel,
remember that with STARTTLS you can decide to force or not TLS, with
tcpserver/ssl i dont see how you may do that.
