Something very non-Qmail-Scanner related is wrong with your setup. qmail-smtpd calls qmail-scanner-queue.pl via the QMAILQUEUE environment variable, and after processing the message, Q-S calls qmail-queue itself. It NEVER calls qmail-smtpd - which is what your pstree shows. Something is very broken on your system.
