My system: qmail 1.03, RH 8 (kernel 2.4.20-18.8), qmail-scanner 1.16, clamav 0.54; patches: Bruce Guenter's qmailqueue patch.
I am planning on setting up my qmail box as an antivirus gateway in a setup like this (view in fixed-width font):

qmail with qmail-scanner are running correctly. Test e-mail is getting delivered to local accounts on the box, and outgoing email works too. Viruses are getting caught with clamscan, and both the sender and recipient are getting notified. At this point, I have no forwarding setup.
So to setup forwarding to our other mail server, I added "macalester.edu" to /var/qmail/control/rcpthosts, and it is NOT in locals. I also then added "macalester.edu:mail.macalester.edu" to the /var/qmail/control smtproutes file. I restart qmail with 'qmailctl restart'.
E-mail gets forwarded, WITHOUT the body. Everything else about the email looks a-ok, but there is just no body. To troubleshoot this, I took qmail-scanner out of the equation. I changed my /etc/tcp.smtp file so it doesn't use the qmail-scanner-queue anymore to the default:
127.:allow,QMAILQUEUE="/var/qmail/bin/qmail-queue"
:allow,QMAILQUEUE="/var/qmail/bin/qmail-queue"
I then do a 'qmailctl cdb' to have it re-read the file. Voila, forwarding now works and the bodies are included.
So it looks like I have something configured incorrectly regarding forwarding AND qmail-scanner at the same time. Does anyone know what?
