2 things:

* check permissions. Q-S runs as qmailq - no doubt you tested it as root...

* are you  running Q-S under
  softlimit with the max RAM setting too low. It could be that sweep is
  crashing (out of memory) but not reporting it - although that's really
  doubtful that it wouldn't report it.
