Thanx for the response. Yes I am using per
IP basis selective relaying for In-to-Out mails. But atleast I don’t know
what IP will be of a user sending mails to my domain, Out-to-In, let say from a
German domain or from Hotmail…..
Any other clue?

you can do this with tcp.cdb file
for user you don't want to be scanned just add rule in tcp.cdb with IP address
for example
192.168.1.23:allow,RELAYCLIENT="",RBLSMTPD="",QMAILQUEUE="/var/qmail/bin/qmail-queue"
secret is in QMAILQUEUE which is original one in the example
if QMAILQUEUE is "qmail-scanner-queue.pl" mail will be scanned,
if QMAILQUEUE is "qmail-queue" you will turn off qmail-scanner for
this IP address
I don't know how you can made it per user, but per IP address there is it
