Yup. If someone makes a SMTP connection, and waits until it times out,
qmail-queue/qmail-scanner is still called by qmail-smtpd. But as there is no
envelope sender or recipient (well, recipient is all that matters really),
then they just clean up and exit.

This is a Qmail question - not Q-S really. Again, it's when a SMTP session
is killed for whatever reason half way through. e.g. the SMTP client sends
99% of the message, and then the (client) server dies due to a disk failure.
You end up with half-written files on the (Q-S) server. Again, it's cleaned
up as it was a temp failure.

I haven't seen this with Q-S. Maybe it is vpopmail - I wouldn't know. BTW it
is possible to get double-delivery with SMTP - but it should be very rare.
