Well originally I assumed they were just incorrectly formatted spam emails
also, but when I examined some, they were not being flagged as such, with
a SA score of 1 or 2.

But it must fiddle to (a) insert X-Spam-Status headers and (b) change the
subject line. I take your point that it isn't widely reported with Q-S
users though and perhaps I just have set something up incorrectly

It seemed to me to be the most likely cause, but I have no other reason
really. I think I'll bark up another tree for a bit and see what turns up
