The inetd from RedHat Linux 4.2, 5.0 and 5.1 can be configured to allow
higher connection rates. So can inetd from at least half the Amiga TCP/IP
stacks. One of our (k-net.dk) mail relays still launces qmail-smtpd from
inetd and has done so all the time (which is approaching two years). This has
worked fine since the connection rate limit was incresed from the default 10
(or something like that) per minute to something appropriate.
