Incoming:
Put your own domains into control/rcpthosts and control/locals
and use the alias/.qmail-default mechanism to forward them all
to your internal mailserver. See qmail-smtpd (8)

To 'rewrite' the domain only with alias/.qmail-default use
something like this: (not shure but somebody will correct me...)

&${LOCAL}@intranet.lan

and use control/smtproutes for difficult cases or if your
intranet.lan does not have an MX etc.


Outgoing:
Put your own IP's into control/relayclients and qmail will do
the work for you. See qmail-remote (8)
Configure qmail as smarthost at your internal mailserver.
