As long as you make sure the $mailprog variable eventually invokes qmail's
sendmail hook, it works fine. You can either alter the
$mailprog = '/usr/lib/sendmail';
line to read
$mailprog = '/var/qmail/bin/sendmail';
or make /usr/lib/sendmail a symlink to /var/qmail/bin/sendmail. Of course,
if you've already done the latter, you should be able to just drop formmail
onto your system and have it work like a champ.
