It is added on maildir and mailbox delivery. When delivering to a
program, it is not added automatically, but is available in an
environment variable ($RPLINE) for the program to use if it so wishes.
When forwarding, it is (of course) not added, but the envelope sender
is preserved, so it can be duly noted on final delivery.
If you find any departures from these rules, let us know the details.
