I think maybe you forgot 'qmail-smtpd'.. :)
try to load it : /usr/local/bin/tcpserver -c100 -u 65534 0 smtp
/var/qmail/bin/qmail-smtpd &
or add 'qmail-smtpd' into /etc/inetd.conf.
