Who needs that, anyway? ;)

Can't get you any BSD specific examples, but here's what I've got on a dial-up linux:

qmail/control:
me

my hostname (if you have dynamic IPs, setting up your smtp server's hostname should
work perfectly)

That's also exactly what I use.
I start qmail with:
qmail-start '|preline procmail' splogger qmail
And my sendmail binary (mutt is configured to use sendmail for outgoing mail) is
actually the wrapper that comes with qmail.
As for fetchmail, here's what I've got:
~/.fetchmailrc
-----
set syslog
set postmaster "rmcc"
set bouncemail
set properties ""
set daemon 600
poll pop.isp.tld with proto POP3
user "username" there with password "password" is rmcc here options fetchall
warnings 3600 mda "/usr/bin/procmail -d rmcc"
antispam 571 550 501 554

If you run qmail-start like I do, it will use the "common" spool/mail/$USER, because
it'll use procmail.
