Issues with qmail pop3d and $HOME/Mailbox

I have been unable to find out why I cannot get past the error:
* ERR this user has no $HOME/Maildir
Connection closed by foreign host.
I have tried everything I have found on the docs page and FAQ and still
cannot figure out where I have gone wrong.
Please help before I pull out what little hair I have left.
