Actually they have. A couple of us even implemented patches to do some of
the things that people suggest. The multiple recipient issue came up
and was discussed when qmail was in beta. I recall raising it with others
around the 0.76 release which was almost four years ago. Throttling the
number of connections to a given site was another issue discussed very
early on and at least one person on the list at the time made patches
available to do that.
What happened with me is that I worried about it for years and even saw
some of the pathological situations that people refer to (and having worked
on the mail systems of numerous non-US ISPs that pay and charge by the bit
only exacerbates that concern). But time and observations eased those
concerns and in the end the cost/benefit to make the changes have never
materialized.
