Are you running a GENERIC kernel? What is maxusers set to? See
/sys/i386/conf/<kernelname> for its value (GENERIC if you are running the
GENERIC kernel).

Btw, 2.2.[78] is ancient. I suggest you look into upgrading to 4.1.1, the
latest stable release.
