It's for creating local email addresses that don't go to real users. In particular you *must* handle root that way; qmail will not deliver directly to root as a security measure (since qmail-local runs as the user being delivered to). Postmaster is also usually handled through an alias, though it would be perfectly possible to create a postmaster user instead if that worked better for your needs.
