please use the qmail-ldap mailing list for qmail-ldap related stuff.
You'll need a clean qmail-1.03 tree to succeed.
tar xzf qmail-1.03.tar.gz
cd qmail-1.03
lynx --source http://www.nrg4u.com/qmail/qmail-ldap-1.03-20010101.patch.gz \
| gunzip -c | patch -p1
(the lynx...patch stuff is one line)
