you need a line like:
127.:allow,RELAYCLIENT=""
In your tcp.smtpd file (which you then build into your tcp.smtpd.cdb)
I've put these files in /etc so to rebuild on my box requires:
/etc/tcp.smtp.cdb /etc/tcp.smtp.tmp < /etc/tcp.smtp
Which means that tcpserver will set the RELAYCLIENT ENV variable - this
is then allows qmail to relay the mail.
Does this help ?
