Would that require running multiple IPs on the box with an instance of
qmail-smtpd running on each IP?

Run multiple copies of qmail on the same machine.  Cause the perl
script to choose one of them at random, by running one of:

/var/qmail1/bin/qmail-inject
/var/qmail2/bin/qmail-inject
/var/qmail3/bin/qmail-inject
/var/qmail4/bin/qmail-inject
