I've read not everything and as usual the docs are a lot indeed!
When try to reply and the domain is not into ../rcpthost I get an error.
I start my qmail from inetd using the line:
smtp stream tcp nowait qmaild /var/qmail/bin/tcp-env tcp-env
/var/qmail/bin/qmail-smtpd
