Perhaps it has a message larger than 10485760 bytes for you. Note that
stock qmail-smtpd doesn't use EHLO SIZE, so either try a stock
qmail-smtpd or find some other way to cause qmail-smtpd to not spit
out EHLO SIZE.

I speculate that Lotus Domino is probably head-of-line blocking on
this one oversize message, preventing smaller messages from getting
through.
