I bet you don't have a control/rcpthosts file.

I was having the same problem.  I don't know if this is a qmail problem,
or if it's a qmail-ldap problem (I'm using an old qmail-ldap patch, so
this might be even fixed).

Try creating a control/rcpthosts file, ok?
