To my knowledge, their only criteria is whether the recipient address
appears in the To: or cc: headers.  If it doesn't (mailing list mail,
bcc'd mail), it's junk mail.

It's possible they're also doing something based on the originating MX
(RBL/ORDB/ORBZ/etc).

The hotmail address doesn't appear in the To: header -- ergo, it's junk
mail.
