big-todo patch and reiserfs

Since I have installed qmail from Bruce Guenter's RPM I wonder what
changes the big-todo patch makes to qmail.
Is the vanilla qmail queue/todo a flat directory?
If it is - does the big-todo patch make any sense on reiserfs?
