You have to restart qmail-send (i.e. qmail, not qmail-smtpd), as

       and restart qmail-send.  Exception: If qmail-send receives

Depends on your setup. We do backup MX for customers that turn their
mailservers off during weekends. Thus 48 hours would be too short in OUR
environment. I personally wouldn't make it shorter than at least 4 days.
If you have a setup like ours you may want to stay with the default
(604800 seconds = 7 days) taking in account that you have e.g.
   Sat, Sun, Xmas eve, Xmas holiday 1, Xmas holiday 2  (in Germany)
that would make at least 5.5 days, where some of our customers would have
their mailservers down.
